Ayushman Card List Check

  1. Visit the official Ayushman Card portal of https://beneficiary.nha.gov.in/,
  2. Select your state and district as a beneficiary on the portal and select the name of your tehsil and village.
  3. The village list will open. To check your name, first of all you can see the names one by one in the list or search by family ID or Aadhaar number.
  4. You can also search by entering Aadhaar number and can also search by entering family ID,
  5. Otherwise you can see the names of family members in the complete list,
  6. Through this process, you can see the list of yourself and your village from Ayushman Bharat Portal,

How to Apply Ayushman Card Online?

1st Step :- Login Process

  1. Click on the official website of PM-JAY click here
  2. Click on the Beneficiary option.
  3. Enter your mobile number.
  4. Enter OTP.
  5. Enter the captcha code and click on Login.

2nd Step :- Selecting Process

  1. Select your state name
  2. Select the name of the scheme.
  3. Select the name of your district.
  4. Search By(Family ID, Aadhaar Number, Name, Location, PMJAY ID)
  5. Enter your (Family ID, Aadhar Number, Name, Location, PMJAY ID) number.

3rd Step :- E-KYC Process

  1. Click on the Action column in front of your name.
  2. Select the type of e-kyc (Aadhar OTP, Finger Print, IRIS Scan)
  3. Click on Aadhaar OTP option.
  4. Verify your Aadhar card and click on Allow.
  5. Now OTP will be sent to the registered mobile number linked to your Aadhaar.
  6. Enter Aadhaar registered OTP.
  7. Now your Aadhar and Ration Card details will open.
  8. Now you have to capture your photo from your mobile and upload the photo.
  9. Fill other information (Enter mobile number, date of birth, pin code and select your relationship, area-rural/city, tehsil and village name)
  10. Click on Submit option.
